Enrolment Forms

When completing an Application Form, please include a Data Collection Form and Parish Priest Reference Form together with a copy of the child's Birth Certificate, Immunisation History Statement (available from myGov website), Baptism Certificate and proof of citizenship/visa if relevant.

An Application Fee of $30 is payable when lodging an Application Form.
